Researchers must perfect the dissection technique before carrying out experiments involving cochlear explants. Hair cells are commonly damaged during dissections performed early on in the learning curve, and a particularly problematic moment for their integrity is the removal of the tectorial membrane, which requires steady hands, proper tools, and experience. Sensorineural Hearing Loss (SNHL) reflects damage to the inner ear or ascending auditory pathway. While hearing loss is the most common sensory deficit in humans1, curative therapies do not yet exist2. Although cochlear or auditory brainstem implants can restore some degree of hearing to patients with severe to profound SNHL, the hearing provided by these devices is still very different from &#34;natural&#34; hearing, especially during attempts to understand speech in noise or to listen to music.
While hair cell degeneration has long been considered the primary consequence of traumatic auditory events (e.g., exposure to loud noise), there is growing evidence that the synapses transmitting information from hair cells to the auditory nerve are at least as vulnerable to acoustic trauma3,4,5,6. Since human audiometric thresholds, the current gold standard for the evaluation of hearing function, do not predict specific cellular damage in the inner ear, more refined tools are needed to detect cellular degeneration as soon as possible and to initiate adequate treatment7.
Promising pharmaceutical treatments for hearing loss are often tested on homogenous cell cultures in vitro, but such systems do not accurately model the cochlear microenvironment. Cochlear cells are known to secrete trophic factors that influence other cell types within the cochlea8,9, a crucial in vivo process that is lost when the organ of Corti10,11 or Spiral Ganglion Neurons (SGNs)12 are cultured in isolation or when molecular markers are analyzed13. However, in vivo studies that may be necessary for the validation of in vitro data to establish new, personalized treatments for hearing loss in the pursuit of &#34;precision medicine&#34; require significant resources and time. This is especially relevant when considering how much effort is required to perfect and perform middle ear or round window membrane injections with hearing tests and the subsequent dissection of cochlear whole-mounts. The efficient screening of promising compounds in organotypic ex vivo cultures known as cochlear explants provides an economic and reliable alternative14,15,16,17.
This article details a protocol by which to generate, maintain, and evaluate treated cochlear explants. Specific applications for this model are emphasized, including its use in the screening of potentially therapeutic compounds and the comparative evaluation of viral vectors for gene therapy. An ex vivo explant approach allows researchers to visualize the effects of a given treatment on different cell populations in situ, facilitating the identification of cell type-specific mechanisms and the subsequent refinement of targeted therapeutics.
Overall, this technique provides a model to study the cochlea ex vivo while preserving vital cross-talk between the vastly different cell types that coexist within the cochlea.

While there have been remarkable advances in hearing research over the past few decades, there is still no cure for Sensorineural Hearing Loss (SNHL), a condition that typically involves damage to or loss of the delicate mechanosensory structures of the inner ear. Sophisticated in vitro and ex vivo assays have emerged in recent years, enabling the screening of an increasing number of potentially therapeutic compounds while minimizing resources and accelerating efforts to develop cures for SNHL. Though homogenous cultures of certain cell types continue to play an important role in current research, many scientists now rely on more complex organotypic cultures of murine inner ears, also known as cochlear explants. The preservation of organized cellular structures within the inner ear facilitates the in situ evaluation of various components of the cochlear infrastructure, including inner and outer hair cells, spiral ganglion neurons, neurites, and supporting cells. Here we present the preparation, culture, treatment, and immunostaining of neonatal murine cochlear explants. The careful preparation of these explants facilitates the identification of mechanisms that contribute to SNHL and constitutes a valuable tool for the hearing research community.

While many protocols focus on organ of Corti explants, this technique attempts to preserve the anatomy of the entire cochlear turn, including the SGNs. The overall goal of this procedure is the preparation, culture, treatment, and immunostaining of neonatal murine cochlear explants to be used as an in vitro screening tool in hearing research. This method can help answer key questions in the hearing research field, such as enabling the simultaneous study of cochlear hair cells, synapses, neurites, and neurons in an ex vivo model and providing insight into mechanisms of action of molecules new to the cochlea, including factors found in human secretions. The main advantage of this technique is that the screening process of candidate therapeutic compounds can be accelerated prior to their testing in vivo.Begin with a freshly collected neonate head contained in 70%ethanol in the lid of a Petri dish. Use a scalpel blade to make a superficial cut in the skin from the anterior to the posterior end of the skull. Then cut both external auditory canals and fold the skin anteriorly to expose the whole cranium.Next, open the cranium along the sagittal suture by gently moving the scalpel blade up and down from anterior to posterior to avoid compression of the cochlea. Remove and discard the snout by making a vertical cut directly posterior to the orbits. The posterior part of the skull should still contain the brain and cochlea.Then use 4 forceps to bluntly dissect and dispose of the forebrain, cerebellum, and brain stem. Place the two halves of the cranium into a 60 millimeter plastic Petri dish filled with cold HBSS. Then use 4 forceps to completely expose the cochlea which is adjacent to the surrounding stapedial artery and bluntly separate the organ from the temporal bone.Transfer the cochlea into a 50 millimeter clear walled glass bottom Petri dish containing HBSS and position the dish under the microscope. Place the 60 millimeter plastic Petri dish with the other half of the cranium on ice. Next, use 4 forceps to carefully dissect the cochlear otic capsule.Now, use a micro knife or two 55 forceps to carefully separate the spiral ligament that is adhered to the organ of Corti from the rest of the cochlea and the medialis. This can be done by grasping the spiral ligament by the most basal aspect and gently unwinding it while moving apically. After the spiral ligament is removed, position the specimen to obtain a longitudinal sagittal view of the cochlea.Then cut the cochlea into two or three sections using the micro knife, classifying these sections as the apical and basal turns. Remove the tissue superior for the more apical piece and inferior for the more basal piece, down to the actual layer of hair cells and neurites to allow the cochlear explant to lay horizontally on the Petri dish. Next, use forceps to gently grasp and peel away the tectorial membrane, a very thin translucent layer immediately superior to the organ of Corti.The most technically challenging step is likely the removal of the thin translucent tectorial membrane as one can easily damage the hair cells underneath. By starting from the edges of the tissue, hair cells along the length of the explant are preserved, except at the very edge where peeling of the tectorial membrane began. Lastly, remove Reissner's membrane immediately superior to the spiral ganglion neurons by touching it with forceps on both sides and peeling it away.The cochlear specimen is now ready for culturing after removing any damaged areas on the edges. Begin the culture procedure by wetting the tip of a one milliliter pipette with approximately 120 microliters of DMEM to prevent the explant from adhering to the inside of the tip. Then use the pre-wetted tip to pipette a maximum of 70 microliters from the glass bottom Petri dish and place the specimen along with the liquid on one of four inlays in the four-well Petri dish.Check under the microscope that the explant is in the correct orientation. Ensure that the area of hair cells from which the tectorial membrane was removed faces upwards and that the basilar membrane together with the trimmed basal part of the medialis faces downwards and adheres to the coverslip inlay. If the specimen is orientated upside down upon inspection, as shown here, add HBSS from the pipette tip and reposition the piece until it is correctly oriented.Once the explant is correctly oriented, aspirate excess HBSS then wait for 15 seconds. Now, pipette 60 microliters of warm culture medium directly unto the specimen, taking care not to touch the specimen with the pipette. Replace the inner and outer Petri dish covers and incubate overnight at 37 degrees Celsius.The next day, transfer the cochlear explant containing Petri dishes from the incubator to the laminar flow hood. Aspirate the old culture medium which does not contain FBS from the inlay. Add 60 microliters per well of the prepared warm treatment medium.Place the Petri dish back into the incubator until staining of the cultures after a maximum of seven days. As seen in this confocal image, this technique preserves the anatomy of the entire cochlear turn including the spiral ganglion neurons or SGNs, allowing analysis of treatment effects on neurites and somata of SGNs in addition to the organ of Corti. As seen here, these explants can be used to simultaneously compare the effect of treatment on apical and basal turns after immunofluorescent staining.Hair cells are shown in green and neuronal structures appear red. Intact synapses can be assessed by labeling pre-synaptic proteins shown in red and post-synaptic proteins shown in green together with the staining of neuronal structures in blue. This figure shows representatives images of apical and basal cochlear explants after seven days in culture with either 3%FBS on the left or 1%FBS on the right.Cells incubated in 3%FBS start to migrate between days four and five as shown by the light gray arrows for hair cells and the red arrows for neurites. Explants in 1%FBS maintain organizational integrity until day seven. While attempting this procedure, it's important to remember to visually ensure the structural integrity and orientation of the cochlea tissue before adding the final culture media and substances of interest.Following this procedure, 3D reconstruction can be performed to confirm cell type specific localization of GFP expressing cells within the medialis.

Making Sense of Listening: The IMAP Test Battery

The ability to hear is only the first step towards making sense of the range of information contained in an auditory signal. Of equal importance are the abilities to extract and use the information encoded in the auditory signal. We refer to these as listening skills (or auditory processing AP). Deficits in these skills are associated with delayed language and literacy development, though the nature of the relevant deficits and their causal connection with these delays is hotly debated.
When a child is referred to a health professional with normal hearing and unexplained difficulties in listening, or associated delays in language or literacy development, they should ideally be assessed with a combination of psychoacoustic (AP) tests, suitable for children and for use in a clinic, together with cognitive tests to measure attention, working memory, IQ, and language skills. Such a detailed examination needs to be relatively short and within the technical capability of any suitably qualified professional. Current tests for the presence of AP deficits tend to be poorly constructed and inadequately validated within the normal population. They have little or no reference to the presenting symptoms of the child, and typically include a linguistic component. Poor performance may thus reflect problems with language rather than with AP. To assist in the assessment of children with listening difficulties, pediatric audiologists need a single, standardized child-appropriate test battery based on the use of language-free stimuli.
We present the IMAP test battery which was developed at the MRC Institute of Hearing Research to supplement tests currently used to investigate cases of suspected AP deficits. IMAP assesses a range of relevant auditory and cognitive skills and takes about one hour to complete. It has been standardized in 1500 normally-hearing children from across the UK, aged 6-11 years. Since its development, it has been successfully used in a number of large scale studies both in the UK and the USA. IMAP provides measures for separating out sensory from cognitive contributions to hearing. It further limits confounds due to procedural effects by presenting tests in a child-friendly game-format. Stimulus-generation, management of test protocols and control of test presentation is mediated by the IHR-STAR software platform. This provides a standardized methodology for a range of applications and ensures replicable procedures across testers. IHR-STAR provides a flexible, user-programmable environment that currently has additional applications for hearing screening, mapping cochlear implant electrodes, and academic research or teaching.

A test battery (IMAP) for performing an in-depth assessment of auditory and cognitive abilities contributing to listening skills is described. It is quick to administer, child-friendly and free from linguistic confounds. Stimulus generation and protocol management are controlled via a software platform (IHR-STAR) to ensure replicable procedures.

A Strategy to Identify de Novo Mutations in Common Disorders such as Autism and Schizophrenia

There are several lines of evidence supporting the role of de novo mutations as a mechanism for common disorders, such as autism and schizophrenia. First, the de novo mutation rate in humans is relatively high, so new mutations are generated at a high frequency in the population. However, de novo mutations have not been reported in most common diseases. Mutations in genes leading to severe diseases where there is a strong negative selection against the phenotype, such as lethality in embryonic stages or reduced reproductive fitness, will not be transmitted to multiple family members, and therefore will not be detected by linkage gene mapping or association studies. The observation of very high concordance in monozygotic twins and very low concordance in dizygotic twins also strongly supports the hypothesis that a significant fraction of cases may result from new mutations. Such is the case for diseases such as autism and schizophrenia. Second, despite reduced reproductive fitness1 and extremely variable environmental factors, the incidence of some diseases is maintained worldwide at a relatively high and constant rate. This is the case for autism and schizophrenia, with an incidence of approximately 1% worldwide. Mutational load can be thought of as a balance between selection for or against a deleterious mutation and its production by de novo mutation. Lower rates of reproduction constitute a negative selection factor that should reduce the number of mutant alleles in the population, ultimately leading to decreased disease prevalence. These selective pressures tend to be of different intensity in different environments. Nonetheless, these severe mental disorders have been maintained at a constant relatively high prevalence in the worldwide population across a wide range of cultures and countries despite a strong negative selection against them2. This is not what one would predict in diseases with reduced reproductive fitness, unless there was a high new mutation rate. Finally, the effects of paternal age: there is a significantly increased risk of the disease with increasing paternal age, which could result from the age related increase in paternal de novo mutations. This is the case for autism and schizophrenia3. The male-to-female ratio of mutation rate is estimated at about 4&#x2013;6:1, presumably due to a higher number of germ-cell divisions with age in males. Therefore, one would predict that de novo mutations would more frequently come from males, particularly older males4. A high rate of new mutations may in part explain why genetic studies have so far failed to identify many genes predisposing to complexes diseases genes, such as autism and schizophrenia, and why diseases have been identified for a mere 3% of genes in the human genome. Identification for de novo mutations as a cause of a disease requires a targeted molecular approach, which includes studying parents and affected subjects. The process for determining if the genetic basis of a disease may result in part from de novo mutations and the molecular approach to establish this link will be illustrated, using autism and schizophrenia as examples.

Excitotoxic Stimulation of Brain Microslices as an In vitro Model of Stroke

Examining molecular mechanisms involved in neuropathological conditions, such as ischemic stroke, can be difficult when using whole animal systems. As such, primary or &#39;neuronal-like&#39; cell culture systems are commonly utilized. While&#160;these systems are relatively easy to work with, and are useful model systems in which various functional outcomes (such as cell death) can be readily quantified, the examined outcomes and pathways in cultured immature neurons (such as excitotoxicity-mediated cell death pathways) are not necessarily the same as those observed in mature brain, or in intact tissue. Therefore, there is the need to develop models in which cellular mechanisms in mature neural tissue can be examined. We have developed an in vitro technique that can be used to investigate a variety of molecular pathways in intact nervous tissue. The technique described herein utilizes rat cortical tissue, but this technique can be adapted to use tissue from a variety of species (such as mouse, rabbit, guinea pig, and chicken) or brain regions (for example, hippocampus, striatum, etc.). Additionally, a variety of stimulations/treatments can be used (for example, excitotoxic, administration of inhibitors, etc.). In conclusion, the brain slice model described herein can be used to examine a variety of molecular mechanisms involved in excitotoxicity-mediated brain injury.

Dissection and Explant Culture of Murine Allantois for the In Vitro Analysis of Allantoic Attachment

The placenta is essential for the growth and development of mammalian embryos. For this reason, numerous genetic alterations and likely also environmental insults that disturb placenta development or function can cause early pregnancy loss in mice and humans. Nevertheless, simple in vitro assays to screen for potential effects on placenta formation are lacking. Here, we focus on modeling the first and critical step in placenta formation, which consists of the attachment of the allantois to the chorion. We describe a method to rapidly assess the attachment of allantoic explants on immobilized &#945;4&#946;1 integrin, which serves as a chorio-mimetic substrate.This in vitro approach enables a qualitative evaluation of the attachment and spreading behavior of multiple allantois explants at different consecutive time points. The protocol may be used to investigate the effect of targeted mouse mutations, drugs, or various environmental factors that have been linked to pregnancy complications or fetal loss on allantois attachment ex vivo.

Vagus Nerve Stimulation As an Adjunctive Neurostimulation Tool in Treatment-resistant Depression

Vagus nerve stimulation (VNS) is an approved neurostimulation therapy. The purpose of the method is to treat patients with therapy-resistant depression (TRD). VNS exhibits antidepressive and stabilizing effects. This method is particularly useful as a long-term treatment, in which up to two-thirds of patients respond. The vagus nerve stimulator is positioned on the left vagus nerve during a surgical procedure and is activated telemetrically by a wand connected to a handheld computerized device. The treating physician can perform various adjustments of the vagus nerve stimulator during in-office visits (e.g., by modifying stimulation intensity or stimulation frequency) to achieve maximum therapeutic effects with low side effects. Set-up of the device usually takes several months. Typical side effects include wound infection, temporary salivation, coughing, paralysis of the vocal cords, bradycardia, or even asystole. The patient can stop the VNS by placing a magnet over the generator. The current protocol describes delivery of the specific stimulation tool and methods for adjusting the tuning parameters to achieve the best remission rates in patients with TRD.

Murine Precision-Cut Liver Slices as an Ex Vivo Model of Liver Biology

Understanding the mechanisms of liver injury, hepatic fibrosis, and cirrhosis that underlie chronic liver diseases (i.e., viral hepatitis, non-alcoholic fatty liver disease, metabolic liver disease, and liver cancer) requires experimental manipulation of animal models and in vitro cell cultures. Both techniques have limitations, such as the requirement of large numbers of animals for in vivo manipulation. However, in vitro cell cultures do not reproduce the structure and function of the multicellular hepatic environment. The use of precision-cut liver slices is a technique in which uniform slices of viable mouse liver are maintained in laboratory tissue culture for experimental manipulation. This technique occupies an experimental niche that exists between animal studies and in vitro cell culture methods. The presented protocol describes a straightforward and reliable method to isolate and culture precision-cut liver slices from mice. As an application of this technique, ex vivo liver slices are treated with bile acids to simulate cholestatic liver injury and ultimately assess the mechanisms of hepatic fibrogenesis.

This protocol provides a simple and reliable method for the production of viable precision-cut liver slices from mice. The ex vivo tissue samples can be maintained under laboratory tissue culture conditions for multiple days, providing a flexible model to examine liver pathobiology.

Measurement of Strial Blood Flow in Mouse Cochlea Utilizing an Open Vessel-Window and Intravital Fluorescence Microscopy

Transduction of sound is metabolically demanding, and the normal function of the microvasculature in the lateral wall is critical for maintaining endocochlear potential, ion transport, and fluid balance. Different forms of hearing disorders are reported to involve abnormal microcirculation in the cochlea. Investigation of how cochlear blood flow (CoBF) pathology affects hearing function is challenging due to the lack of feasible interrogation methods and the difficulty in accessing the inner ear. An open vessel-window in the lateral cochlear wall, combined with fluorescence intravital microscopy, has been used for studying CoBF changes in vivo, but mostly in guinea pig and only recently in the mouse. This paper and the associated video describe the open vessel-window method for visualizing blood flow in the mouse cochlea. Details include 1) preparation of the fluorescent-labeled blood cell suspension from mice; 2) construction of an open vessel-window for intravital microscopy in an anesthetized mouse, and 3) measurement of blood flow velocity and volume using an offline recording of the imaging. The method is presented in video format to show how to use the open window approach in mouse to investigate structural and functional changes in the cochlear microcirculation under normal and pathological conditions.

Performing Intracochlear Electrocochleography During Cochlear Implantation

Electrocochleography (ECochG) measures inner ear potentials generated in response to acoustic stimulation of the ear. These potentials reflect the residual function of the cochlea. In cochlear implant candidates with residual hearing, the implant electrode can directly measure ECochG responses during the implantation process. Various authors have described the ability to monitor the inner ear function by continuous ECochG measurements during the surgery. The measurement of ECochG signals during surgery is not trivial. There are no interpretable signals in up to 20% of cases. For a successful recording, a standardized procedure is recommended to achieve the highest measurement reliability and avoid possible pitfalls. Therefore, seamless collaboration between the CI surgeon and CI technician is key. This video consists of an overview of the system setup and a stepwise procedure of performing intracochlear ECochG measurements during CI surgery. It shows the surgeon's and the CI technician's roles in the process, and how a smooth collaboration between the two is made possible.

Modified Technique for the Use of Neonatal Murine Hearts in the Langendorff Preparation

The use of the ex-vivo retrograde perfused heart has long been a cornerstone of ischemia-reperfusion investigation since its development by Oskar Langendorff over a century ago. Although this technique has been applied to mice over the last 25 years, its use in this species has been limited to adult animals. Development of a successful method to consistently cannulate the neonatal murine aorta would allow for the systematic study of the isolated retrograde perfused heart during a critical period of cardiac development in a genetically modifiable and low-cost species. Modification of the Langendorff preparation enables cannulation and establishment of reperfusion in the neonatal murine heart while minimizing ischemic time. Optimization requires a two-person technique to permit successful cannulation of the newborn mouse aorta using a dissecting microscope and a modified commercially available needle. The use of this approach will reliably establish retrograde perfusion within 3 min. Because the fragility of the neonatal mouse heart and ventricular cavity size prevents direct measurement of intraventricular pressure generated using a balloon, use of a force transducer connected by a suture to the apex of the left ventricle to quantify longitudinal contractile tension is necessary. This method allows investigators to successfully establish an isolated constant-flow retrograde-perfused newborn murine heart preparation, permitting the study of developmental cardiac biology in an ex-vivo manner. Importantly, this model will be a powerful tool to investigate the physiological and pharmacological responses to ischemia-reperfusion in the neonatal heart.

Effect of Hyaluronic Acid 35 kDa on an In Vitro Model of Preterm Small Intestinal Injury and Healing Using Enteroid-Derived Monolayers

In vitro scratch wound assays are commonly used to investigate the mechanisms and characteristics of epithelial healing in a variety of tissue types. Here, we describe a protocol to generate a two-dimensional (2D) monolayer from three-dimensional (3D) non-human primate enteroids derived from intestinal crypts of the terminal ileum. These enteroid-derived monolayers were then utilized in an in vitro scratch wound assay to test the ability of hyaluronan 35 kDa (HA35), a human milk HA mimic, to promote cell migration and proliferation along the epithelial wound edge. After the monolayers were grown to confluency, they were manually scratched and treated with HA35 (50 &#181;g/mL, 100 &#181;g/mL, 200 &#181;g/mL) or control (PBS). Cell migration and proliferation into the gap were imaged using a transmitted-light microscope equipped for live-cell imaging. Wound closure was quantified as percent wound healing using the Wound Healing Size Plugin in ImageJ.&#160;The scratch area and rate of cell migration and the percentage of wound closure were measured over 24 h. HA35 in vitro accelerates wound healing in small intestinal enteroid monolayers, likely through a combination of cell proliferation at the wound edge and migration to the wound area. These methods can potentially be used as a model to explore intestinal regeneration in the preterm human small intestine.
